Donor Portal

We appreciate your interest in being part of the IRMS Egg Donor Program. Now that you have completed the Preliminary Donor Application we are inviting you to move on to the next phase of the application process by logging into our Donor Portal and completing all sections of this expanded application.

Please log–in using the unique username and temporary password you were provided via email. Once you have successfully logged in, a Disclaimer followed by “Notice of Privacy Practices” will appear. Read through these pages and choose “I agree” if you intend to use this Portal.  If you do not agree you will not be able to fill out the application this way.

If you have agreed, the next page is basic demographic information. After reviewing and completing the Demographic Information, the main Patient Portal page will appear. Demographic information can only be changed on our end. If anything is incorrect let us know and we will correct it. Incorrect information on the demographics page will not prevent you from completing the application at this time.

Next click on “History” and complete all of the questions regarding your medical history contained therein (click “next” at the end of each page to move forward) Please be sure to complete ALL parts of the application as there are many and SAVE as you move through each section.

• Please change your temporary password – go to profile, then click change password to make your own unique password.
• When finished, click on “Log off”.

Once you have completed the Donor Portal send an email notification to CLucus@irms.com that your application is complete.
