Patient Survey

IRMS highly values the input of our patients in developing, maintaining, and improving our services.  As a “learning organization”, we need to hear from our patients in order to know what we are doing right—and where we need to improve.

In order to participate in our online survey, click on the link below.  At the top of the survey, you will be asked to enter a code.  This code is found at the front desk in the Livingston office and the Hoboken office and it changes periodically.  The only reason that we require the entry of this code is to ensure that our surveys are being completed by people that have been in our office and it allows us to know the approximate time period during which they received services.  It does not, in any way, allow us to identify you—you may remain completely anonymous, if you choose.

If you would like to participate in our online survey, but you no longer have access to our office (e.g. you have transferred to your obstetrician, you have elected to discontinue treatment, etc.), please click here and type the words “SURVEY PASSWORD” in the message line of the email.  The code will then be returned to you so that you can participate in the survey.

Our system allows a survey to be submitted online once every three months.  If you attempt to submit a survey more frequently, it will be rejected, based on your computer’s IP address (which is not specifically identifiable to us).
